A myomectomy is the surgical removal of uterine fibroids (or myomas) that allow the patient’s uterus to remain intact.Myomectomy A myomectomy is the preferred treatment option over a hysterectomy in many cases, as it allows the patient to preserve and retain her reproductive potential to bear future children.  However, while a myomectomy may improve the patient’s chances of conceiving, it is not guaranteed.  All fibroids are not created equal and removal is not always necessary.  If the fibroids cause abnormal bleeding, pressure, and/or pain, a myomectomy may be necessary.

Surgical methods to perform a myomectomy depend on the location, size, and number of the fibroids.  If fibroids are located near the outside of the uterus but away from the uterine cavity, a myomectomy may be accomplished via operative laparoscopy.  This is done using an abdominal incision.  Furthermore, fibroids positioned within or very near the uterine cavity may be removed using hysteroscopy.  A hysteroscopic myomectomy is performed by vaginal approach and involves inserting a lighted viewing instrument into the uterus by way of the vagina.  Laparotomy is another typical surgical method used to extirpate fibroids and entails a larger incision into the abdomen.  Laparotomy may be called for in cases that involve the removal of large fibroids or the presence of numerous fibroids.  Patients with additional questions or concerns pertaining to fibroids should consult with their health care provider.
